What do the bell and note on a stop do?

The bell sets the approach alert: alerting stops export as course points, so your head unit warns you as you get close. Mute the bell and the stop stays on the route but exports quietly (as a GPX waypoint only). The note travels with the stop — it becomes the waypoint description on GPX exports.
